RN Case Manager

This position is responsible to develop, implement, and monitor homecare services for PACE North participants. Duties include conducting initial and periodic assessments of participants' home care & safety needs, coordinating care delivery, providing in-home nursing care and prescribed treatments, authorization of durable medical equipment purchases, supervisory oversight of Homecare Aides, and membership on the Interdisciplinary Team.

The ideal candidate will be a licensed in the state of Michigan as a RN with a valid MI driver’s license and current CPR certification. Minimum 2 years recent experience working as a nurse, preferably in home or community health, in addition to 1 year of experience working with a frail or elderly population. Ability to work effectively in a team environment. All employees must demonstrate that they are fully vaccinated for COVID-19 and have a current flu vaccine.
